Home Hospital Program at NYU Langone Hospital—Long Island
NYU Langone’s Home Hospital Program offers in-home care for people who need the services and care that hospitalization provides, but who are stable enough to be monitored in the comfort of their own homes. Patients receive their care through in-person and virtual visits with their care team. To be eligible, patients are screened by a registered nurse and physician at the Emergency Department at NYU Langone Hospital—Long Island.
Patients admitted into the program are then transported home where a registered nurse is waiting. The nurse completes a medical exam, administers medication, completes bloodwork, and performs other aspects of care as needed. Every day, patients receive a physician visit, at least two nurse visits, and vital signs monitoring. These visits may be completed in-person or through a virtual visit. Care is continuously monitored by our team and is available around the clock. A dedicated hospitalist—a physician specializing in hospital-level care—also provides a daily evaluation and coordinates consultations with other specialists as needed.
Our team coordinates all necessary services, including the following:
- skilled nursing
- intravenous medication and delivery of other medications
- physical therapy
- occupational therapy
- laboratory services
- oxygen treatment and respiratory therapy
- durable medical equipment, including walkers and canes
- round-trip transportation for necessary inpatient services
- speech therapy
- ultrasound and X-ray imaging
- electrocardiogram
